Step 1: Decide How You’ll Apply

You can apply:

  • Online via the SSA website (ssa.gov)
  • By phone: 1-800-772-1213
  • In person at your local Social Security office


Applying online is often the fastest and most convenient option, but it can still be confusing — especially if you’re not sure what to say.


Step 2: Gather the Right Information

To complete the application, you’ll need:

  • Your Social Security number
  • Information about your medical conditions
  • Names and contact info for all doctors and clinics

  • Any documents related to workers’ comp, military service, or long-term disability

SSA-16: Application for SSDI

SSA-3368: Adult Disability Report
